Form 6 – Complete Step-by-Step Process
What is Election Form 6?
Form 6 is the official application form used for inclusion of name in the Electoral Roll (Voter List) in India. If a citizen turns 18 years or above and wants to register as a new voter, Form 6 must be submitted.It is governed by the provisions of the Representation of the People Act, 1950 and administered by the Election Commission of India.
Who Should Apply Using Form 6?
You should apply through Form 6 if:
- You have completed 18 years of age
- You are an Indian citizen
- Your name is not already included in any voter list
- You shifted from one constituency to another
- You are registering as a first-time voter
Where to Apply?
You can apply in two ways:
- Online Mode (Recommended)
Through the official portal:
- National Voters’ Services Portal (NVSP)
- Voter Helpline Mobile App (by ECI)
- Offline Mode
Submit Form 6 physically to:
- Booth Level Officer (BLO) or
- Assistant Electoral Registration Officer (AERO) or
- Electoral Registration Officer (ERO)
Online Form 6 Application – Step-by-Step Process
Step 1: Visit Official Website
Go to: https://www.nvsp.in
Click on “Register as a New Voter (Form 6)”
Step 2: Create/Login Account
- Register using mobile number & OTP
- Create password
- Login to dashboard
Step 3: Select Form 6
Choose:
- “Fresh inclusion/Enrollment”
or - “Shifting from another constituency”
Step 4: Fill Personal Details
You must enter:
- Full Name (as per documents)
- Date of Birth
- Gender
- Place of Birth
- Aadhaar Number (optional but recommended)
- Mobile Number & Email ID
Step 5: Upload Required Documents
Documents Required
- Age Proof (Any One)
- Birth Certificate
- 10th Certificate
- PAN Card
- Address Proof (Any One)
- Aadhaar Card
- Driving License
- Passport
- Bank Passbook
- Passport Size Photo
Step 6: Declaration
- Confirm you are not enrolled elsewhere
- Accept declaration
- Submit form
After submission, you will receive a Reference Number for tracking.
How to Track Form 6 Status?
- Visit NVSP website
- Click “Track Application Status”
- Enter Reference Number
Verification will be done by the Booth Level Officer (BLO) through field verification.
Processing Time
- Usually takes 2–4 weeks
- After approval, your name will appear in the Electoral Roll
- You can download your EPIC (Voter ID Card) online
Offline Application Process:
- Collect Form 6 from BLO/ERO office
- Fill manually
- Attach photocopies of documents
- Submit to BLO
- Verification & approval process
Important Points to Remember
- You must apply only in your current residential constituency
- False information may lead to rejection or legal action
- Only one voter registration is allowed per person
- Aadhaar linking is voluntary but useful

- To shift within same constituency → Use Form 8
